Job summary

Pine Park Health, Inc. is expanding its geriatric specialty care in Reno, NV.

The Provider will diagnose and manage primary care patients in our Reno Clinic and in assisted living/residential settings, delivering comprehensive, patient-centered in-home healthcare to seniors across Northern Nevada. You will perform annual health assessments, coordinate care with the team, and ensure timely documentation and reporting.

Qualifications

• Current Nevada NP license and Medicare/Medicaid credentialing
• Minimum two years in geriatrics in clinical or community settings
• Demonstrates excellent communication with patients and families
• Licensure current per state regulations and professional standards
• Active participation in Continuing Medical Education as required

Responsibilities

• Perform annual comprehensive health assessments in clinic or residential settings
• Obtain history, physicals, and perform exams for new and existing patients
• Coordinate with care team and external partners for patient care
• Document assessment findings and care plans in the medical record
• Identify and address care gaps, chronic conditions, and prevention needs
• Provide in-home post-acute visits and transition planning as needed
