Job summary

Pine Park Health is seeking a Part-Time Nurse Practitioner to deliver onsite primary care in our Phoenix market (Glendale/Sun City). You’ll work with a small, complex-patient panel and a multi-disciplinary team to provide high-quality, patient-centered care in senior living communities.

Qualifications

• Nurse Practitioner qualification with geriatrics experience (AGNP preferred).
• Comfortable in a fast-paced, evolving environment.
• Willingness to provide onsite clinical across Glendale/Sun City region.
• Desire to work with complex patient care and holistic model.
• Strong collaboration within a multidisciplinary team.

Responsibilities

• Provide onsite primary care services to older adults living in Senior Living Communities PT 2-3 days per week.
• Develop and execute detailed person-centered care plans on all primary care patients.
• Work with Medical Director to identify and develop policies affecting day-to-day practice and guidelines.
• Provide monthly Chronic Care Management (CCM) visits and sign CCM documentation.
• Run regular huddles and provide leadership to the clinical team.
• Identify ways to work with senior living management to improve patient outcomes.
