Job description

About this position

Pine Park Health is a value-based primary care practice revolutionizing healthcare for senior living community residents. We empower seniors to get healthy, stay healthy, and lead lives they love through a care model designed with everyone in mind — patients, families, community staff, providers, and payers.

Our approach brings healthcare directly to seniors' homes, offering prevention, screening, chronic condition management, lab work, and diagnostic testing in the comfort of their apartments. With weekly community visits, our teams provide consistent care while collaborating closely with facility staff to address comprehensive health needs. We've eliminated unnecessary barriers to urgent care through same/next-day appointments, helping seniors avoid emergency rooms and hospitalizations where they risk exposure to additional health concerns.

Today, over 185+ communities across Arizona, California, and Nevada trust Pine Park Health, and we're rapidly expanding our reach and impact.

If you're mission-driven and passionate about transforming senior healthcare, this is your opportunity to make a meaningful difference!

The Opportunity

The Clinical Director (NP) at Pine Park Health combines clinical excellence with inspiring leadership, balancing direct patient care with provider management responsibilities. This hybrid role offers an experienced Nurse Practitioner a unique opportunity to deliver exceptional care to seniors while developing and leading a talented team of healthcare providers in our innovative in-home care model.

What you'll do:

Clinical Responsibilities

  • Establish meaningful patient relationships through comprehensive Welcome Visits and develop personalized Care Plans covering goals, diagnoses, medications, and advanced care planning.
  • Ensure timely completion of Annual Wellness Visits focused on preventive care and comprehensive documentation.
  • Provide seamless clinical coverage during provider absences, maintaining continuity of care.
  • Conduct patient assessments, medication management, and follow-up care focusing on geriatric-specific needs.
  • Respond promptly to urgent medical and behavioral health concerns to prevent unnecessary hospitalizations.
  • Conduct post-discharge visits for patients transitioning from acute or skilled nursing facilities.
  • Adapt workflows to accommodate urgent care needs while maintaining quality preventive services.

Leadership Responsibilities

  • Lead and develop a team of 6-8 Nurse Practitioners and Physician Assistants through regular performance reviews, mentorship, and professional development
  • Ensure providers meet key metrics, including patient visit targets, documentation timeliness, and care gap closures.
  • Optimize provider schedules and panel assignments.
  • Drive community growth by attending pipeline meetings, leading implementation kickoffs, and building relationships with senior living facility staff.
  • Serve as the primary point of contact for community partners, addressing concerns promptly and fostering collaborative relationships.
  • Lead quality improvement initiatives to enhance patient care and provider efficiency.
  • Participate in the hiring, onboarding, and training new clinical team members.
  • Collaborate with cross-functional teams to improve care delivery systems and clinical workflows.

Qualifications:

  • Active unrestricted DEA with schedules 2, 2N, 3, 3N, 4, 5
  • Master's degree in Nursing with Family or Adult-Gerontology specialization preferred
  • Minimum three years of specialized geriatric experience in clinical, community-based, or senior living settings
  • Minimum 3 years of direct people management experience supervising Nurse Practitioners, Physician Assistants, or similar clinical staff — including performance reviews, managing to metrics, and hiring decisions; informal mentorship or charge nurse experience does not meet this requirement
  • Demonstrates exceptional clinical acumen, including advanced diagnostic skills and comprehensive medical knowledge
  • Exceptional clinical assessment skills with the ability to develop comprehensive care plans and chronic disease management protocols
  • Demonstrated ability to work autonomously in fast-paced, multi-site healthcare environments

Benefits Tailored for You and Your Family:

  • Comprehensive medical, dental, and vision insurance for you and your dependents
  • Flexible spending accounts for health and dependent care
  • Generous PTO and Sick Leave
  • 8 weeks paid parental leave
  • Monthly wellness allowance
  • Company-issued cell phone
  • Annual professional and personal development allowance

Travel is required for this role. You must have the ability to drive between partner facilities regularly.

Total target compensation for this role is up to $224,000, including a base salary range of $175,000 to $195,000, plus an annual performance-based bonus target of 10% to 15% paid out quarterly, equity, and a full benefits package. Final compensation will be shaped by relevant healthcare experience, particularly primary care and geriatrics-specific background, as well as depth of direct people management experience, both weighted equally in final offer decisions. Your recruiter will share more about the full benefits package during the hiring process.
