Psycho Social Therapies, a leading mental health agency serving geriatric residents in nursing homes and assisted living facilities across Ohio, seeks a Licensed Social Worker (LSW, LISW) or Licensed Professional Counselor (LPC, LPCC) to join our team.

You will evaluate clients, create individualized care plans, collaborate with other care providers, monitor progress, maintain confidential records, and provide talk therapy using evidence-based techniques.

Qualifications

• Licensed social worker (LSW/LISW) or licensed professional counselor (LPC/LPCC) in Ohio.

Responsibilities

• Evaluate clients through thorough assessments to identify needs and mental health conditions.
• Create individualized care plans based on evaluations.
• Coordinate with healthcare professionals, nursing staff, and case managers to coordinate services.
• Monitor progress and adjust care plans as needed.
• Maintain confidential records of treatments, interventions, and services provided.
• Provide talk therapy using evidence-based techniques.
