Qualifications

• MD or DO required.
• Board Certified in Family Medicine or Internal Medicine.
• 3–5+ years of post-residency clinical experience.
• Experience caring for geriatric or medically complex populations.
• 2–3+ years of clinical leadership or medical directorship experience.
• California medical licensure or eligibility.

Responsibilities

• Provide comprehensive primary care to geriatric and medically complex patients.
• Conduct new participant evaluations.
• Manage acute and chronic conditions.
• Provide clinical oversight for participants in SNF and ALF settings.
• Participate in interdisciplinary care planning.
• Provide clinical mentorship to physicians and APPs.
• Support provider development and clinical remediation.
• Facilitate clinical education and didactic sessions.
• Support site operations and workflow improvement.
