Qualifications

• Licensed Practical Nurse (LPN) in the state of employment
• Minimum one year of education or experience in geriatric nursing or rehabilitation
• Knowledge of nursing theory and practice, including medication administration
• Ability to read, write and comprehend English
• CPR certified

Responsabilités

• Holds daily shift report with nursing assistants assigned to the unit before the shift.
• Makes rounds on assigned residents at least every two hours.
• Administers medications and treatments per physician orders; orders medications.
• Develops and maintains nursing care plans with Care Plan Coordinator and updates monthly or as conditions change.
• Assists attending physician with rounds and procedures as needed.
• Transcribes and implements physician orders; documents changes.
• Contacts physician and family about condition changes or incidents; notifies supervisor.
• Performs annual evaluations of nursing assistants and documents performance.
• Documents disciplinary concerns for nursing staff and recommends actions.
• Completes required routine documentation and audits.
• Participates in hiring, interviewing and orientation of nursing assistants.
• Participates in Quality Assurance and Performance Improvement (QAPI) processes.
