Job summary

LiveWell Homecare Agency is seeking a Licensed Practical Nurse (LPN) Medication Nurse to provide licensed nursing support in a senior living environment, focusing on safe medication administration and precise clinical documentation.

You will monitor residents, document in MAR/TAR in real time, and collaborate with the care team to update care plans while maintaining regulatory compliance and a compassionate approach to resident well-being.

Qualifications

• Active LPN license in good standing with CPR and First Aid certifications.
• Strong assessment and documentation skills.
• Experience in senior care or medication administration preferred.

Responsibilities

• Administer medications and treatments per physician orders following the Six Rights.
• Document in MAR/TAR in real-time and transcribe orders to support care plans.
• Monitor residents for symptoms; administer meds for relief and report adverse reactions.
• Perform SHUs and observe changes; report findings to the care team.
• Maintain medication carts, complete narcotic counts, ensure regulatory compliance.

Key Responsibilities:

• Administer medications and treatments according to physician orders, adhering to the Six Rights of Medication Administration.
• Accurately document in MAR/TAR in real-time and transcribe physician orders to support care plan updates.
• Monitor residents for symptoms, administer medications for symptom relief, and report any refusals or adverse reactions promptly.
• Conduct Service & Health Updates (SHUs), observe physical, emotional, or cognitive changes, and report findings appropriately.
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulatory standards.

Qualifications & Skills:

• Active LPN license in good standing, CPR, and First Aid certifications.
• Strong assessment and documentation skills.
• Experience in senior care or medication administration preferred.
