Qualifications

Active and unrestricted LPC license in the applying state.
Excellent verbal and written communication in English.
Basic proficiency with technology, including EHR systems.

Responsibilities

Conduct individual and group psychotherapy sessions for geriatric patients in subacute or skilled nursing facilities.
Assess, diagnose, and treat emotional and behavioral health conditions.
Develop and implement evidence-based treatment plans.
Collaborate with facility staff and families to support patient care.
Maintain accurate clinical documentation in an EHR.
Monitor patient progress and adjust treatment plans as needed.
