Job summary

Senior Care Therapy, LLC in Reading, PA is hiring Licensed Professional Counselors to provide in-person psychotherapy to the geriatric population in SNFs across NJ, NY, PA, and MD. We support clinicians with billing, credentialing, and pre-certifications, allowing you to focus on care.

Full-time roles offer 32-40 hour weeks, salary ranges from $56,000 to $90,000 annually, a sign-on bonus up to $6,000, and a benefits package including medical/dental/vision and retirement options.

Qualifications

• Active LPC license in state applying for.
• Strong English oral and written communication.
• Proficient with electronic health records (EHR).
• Excellent organization and documentation skills.
• Ability to provide in-person services at facilities, up to 45 minutes.
• Ability to move between rooms and offices within the facility.

Responsibilities

• Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
• Assess, diagnose, and treat emotional and behavioral health conditions.
• Develop and implement evidence-based treatment plans tailored to patient needs.
• Collaborate with facility staff, interdisciplinary teams, and families.
• Maintain accurate clinical documentation in accordance with regulatory standards using the EHR.
• Monitor patient progress and adjust treatment plans as needed.
