Qualifications

• Active and unrestricted Licensed Professional Counselor license in the state applying for.
• Strong communication in English; in-person service at facilities.

Responsibilities

• Conduct individual and group psychotherapy sessions in subacute or skilled nursing facilities.
• Assess, diagnose, and treat emotional and behavioral health conditions (depression, anxiety, adjustment disorders, cognitive decline).
• Develop and implement evidence-based treatment plans tailored to each patient.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate clinical documentation in an EHR and monitor patient progress.
