Job summary

Senior Care Therapy, LLC is hiring Licensed Professional Counselors to provide in-person psychotherapy to the geriatric population in NJ, NY, PA, and MD. This clinician-owned organization handles billing, credentialing, and pre-certifications to allow you to focus on clinical care.

Qualifications

• Active and unrestricted LPC license in the state applying for.
• Effective oral and written communication in English.
• Basic proficiency with technology, including electronic health records (EHR).
• Strong organizational and documentation skills, with attention to regulatory compliance.
• Ability to provide in-person services at assigned facilities, up to 45 minutes.

Responsibilities

• Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
• Assess, diagnose, and treat emotional and behavioral health conditions (depression, anxiety, adjustment disorders, cognitive decline).
• Develop and implement evidence-based treatment plans tailored to each patient.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate and timely clinical documentation in accordance with regulatory standards using EHR.
• Monitor patient progress and adjust treatment plans as needed.
