Job summary

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Prescriptive authority or ability to obtain it as needed.
• AANP, AACN, or ANCC board certification in appropriate NP specialty.
• Current BLS/ACLS/CPR certification.

Responsibilities

• Conduct adult/geriatric health assessments in-home or at patient residences.
• Review medical history, medications, and environment to assess risks.
• Collaborate with physicians and care team to coordinate follow-up.
• Provide education and referrals to optimize health outcomes.
• Document findings clearly following Matrix guidelines.
