Job summary

Fox Rehabilitation is expanding its geriatric PT team and seeks licensed Physical Therapists to deliver one-on-one care in patients' homes or Senior Living Communities in Delaware. You will assess, create individualized plans, and provide evidence-based therapy to improve mobility and independence.

The role emphasizes autonomy, flexible scheduling, and opportunities to incorporate telehealth for greater access and continuity of care with an interdisciplinary team.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer proficiency and EMR familiarity.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in-home or in senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Document therapy sessions in EMR according to standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate.
