Job summary

FOX Rehabilitation is expanding in East Stroudsburg, PA, seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and in senior living settings. You will assess, plan, and implement individualized therapy, aiming to improve mobility and independence.

The role offers autonomy, flexible scheduling, and opportunities to incorporate telehealth for continuity of care, with a signing bonus for qualified full-time candidates.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior communities, etc).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively within FOX's EMR system according to standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
