Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care to older adults in their homes and in senior living communities in the Greater Delray Beach area. You will develop individualized plans of care, perform evaluations, and provide evidence-based therapy to improve mobility and independence.

Qualifications

• Valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program (DPT)
• Basic computer skills and EMR proficiency.
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and functional goals.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access
