Job summary

FOX Rehabilitation in Newtown, CT is expanding its geriatric home‑visit program and seeks licensed Physical Therapists to deliver one‑on‑one care in homes and senior living communities. You will help patients improve mobility, independence, and quality of life through a flexible schedule and patient‑centered approach.

Responsibilities include evaluations, care plans, EMR documentation, and collaboration with PTs, OTs, SLPs, and Fitness Specialists.

Qualifications

• Licensed Physical Therapist in CT or eligible to obtain licensure.
• Degree from an accredited Physical Therapy program.
• Current CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide in-home and senior living PT to older adults.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy and promote safety, mobility, and independence.
