FOX Rehabilitation is seeking a licensed Physical Therapist to join our Elizabethville, PA team, delivering one-on-one geriatric care in patients' homes and senior living communities under the Geriatric House Calls model.

You will evaluate, plan, and implement evidence-based therapies, document in FOX's EMR, and collaborate with an interdisciplinary team. Telehealth options may extend reach and flexibility, with a focus on aging in place and meaningful patient outcomes.

Formación

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsabilidades

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX's EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
