Job summary

Ohio Living is seeking a dedicated Physical Therapist to perform evaluations and treatments for residents across our life plan communities in Ohio. The role emphasizes adherence to physician orders, patient safety, and high-quality care in home health settings.

Responsibilities include developing treatment plans, guiding patients and families, coordinating with the interdisciplinary team, and participating in discharge planning to ensure seamless transitions.

Qualifications

• Current unencumbered license to practice as a Physical Therapist (PT) in Ohio.
• One year experience in hospital, long-term care, or home health preferred.
• New graduates welcome.
• Experience in geriatric and physical therapy patients required.

Responsibilities

• Initiates contact with physician for orders and treatment plan as patient condition requires.
• Performs physical therapy treatments as prescribed by physician, evaluating and treating patients.
• Educates patients, families, and other health team members on exercise techniques and self-care.
• Participates in interdisciplinary team meetings to ensure continuity of care.
• Conducts home visits to implement the Plan of Care and promotes discharge planning.

Ohio State PT license (Current unencumbered license to practice as a Physical Therapist in Ohio)
