Job summary

Qualifications

• Active Ohio PT license required.
• At least 1 year experience in hospital/long-term care/home health preferred; new grads welcome.
• English literacy and valid driver's license required.

Responsibilities

• Perform PT evaluations and treatments per physician orders.
• Develop and implement plans of care with regular progress updates.
• Educate patients and families on exercises and safety.
• Document using EMR and comply with regulations.
• Participate in discharge planning and inter-disciplinary teams.
