Job summary

Ohio Living seeks a licensed Physical Therapist to provide high-quality care in home health settings. You will evaluate and treat geriatric patients, initiate orders with physicians, and document progress in EMR and charts.

New graduates are welcome; prior experience with geriatric populations and knowledge of therapy modalities are preferred. Excellent communication and safety standards are expected.

Qualifications

• Must hold an Ohio PT license and be unencumbered.
• Minimum one year PT experience; new grads welcome.
• Experience with geriatric patients preferred.
• Knowledge of physical therapy modalities required.

Responsibilities

• Initiates physician orders and treatment plan revisions as the patient’s condition requires.
• Performs PT treatments as prescribed and assesses progress toward goals.
• Maintains current patient records per EMR and paper charts.
• Participates in discharge planning and interdisciplinary communication.
• Exhibits a clean, safe treatment environment and equipment maintenance.
