Qualifications

• Licensed PT in Ohio with current, unrestricted license.
• Experience caring for geriatric patients in hospital, long-term care, or home health.
• Ability to educate patients and families on exercises and safety.

Responsibilities

• Initiate physician orders and plan of care updates as patient condition requires.
• Participate in patient OASIS assessments and discharge planning.
• Perform PT treatments per physician orders and adapt plan as needed.
• Modify Plan of Care based on progress toward goals.
• Educate patients and families on exercises, precautions and self-care.
• Collaborate in interdisciplinary team meetings to ensure continuity of care.
• Communicate patient progress and discharge plans with care team and family.
• Conduct home visits to implement the plan of care and improve function.
• Maintain clean, safe treatment areas and equipment.
• Assist in staffing planning and provide guidance to licensed therapists.
