Job summary

Trustwell Living, LLC seeks a licensed nurse to provide direct resident care in a geriatric in-home health setting and supervise resident assistants. You will monitor health status, document changes, and communicate with the Health Services Director to ensure care plans are followed with privacy and safety in mind.

CPR/First Aid certified professionals preferred; responsibilities include medication administration, report changes to physicians, and maintain records and policies in a collaborative

Qualifications

• Licensed nurse with a minimum of one year in-home health or geriatric nursing.
• Ability to relate to residents, families, staff and professionals attending care.
• Ability to communicate well, verbally and in writing, and maintain accurate records.
• Ability to adhere to policies and procedures and apply basic management principles.
• Willingness to improve skills of self and staff and stay professionally active.
• Ability to lift up to 50 pounds occasionally and 30 pounds regularly.
• Maintains appropriate license and certifications (CPR/First Aid).

Responsibilities

• Provide direct resident care per individualized care plan.
• Monitor resident health status and changes of condition.
• Communicate and document changes to Health Services Director.
• Supervise resident assistants and ensure compliance with policies.
• Administer medications and treatments per community policies.
• Ensure implementation of resident care plans and documentations.
• Report changes to physician orders and nursing staff.
• Maintain resident privacy, dignity and safe environment.
• Perform infection control and safety procedures.
• Respond to emergency medical situations and provide interventions.
