Job summary

Trustwell Living, LLC seeks a licensed nurse to provide direct resident care in an in-home health setting, following individualized care plans and monitoring residents for changes in condition. You will document and communicate resident status to the Health Services Director, supervise resident assistants, administer medications per policy, uphold privacy and safety, and ensure compliance with state and federal regulations.

Qualifications

• Must be a Licensed Nurse with at least one year experience in-home health or geriatric nursing.
• Excellent communication skills, verbal and written.
• Ability to document and maintain accurate records.
• Ability to adhere to and communicate policies and procedures.

Responsibilities

• Provides direct resident care per individualized service/care plan.
• Monitors residents for changes in condition and signs/symptoms of illness.
• Communicates and documents resident changes to Health Services Director.
• Assists in supervision of resident assistants.
• Administers medications/treatments per community policies.
