Job summary

Kintegra Health is seeking a Licensed Physical Therapist to provide services to Senior TLC participants within a home and community-based care model in North Carolina. The PT will assess mobility and functional status, design therapeutic exercise programs, and support gait training and ambulation as part of a maintenance therapy plan.

The role requires NC licensure, a valid driver's license, and a background in geriatric care.

Qualifications

• Licensed Physical Therapist in NC with valid NC driver’s license
• Experience in geriatric population preferred
• Demonstrates clinical competency with geriatric population
• Maintains compliance with Senior TLC values and CMS standards

Responsibilities

• Conduct initial and ongoing assessments of participants’ physical condition and functional status.
• Develop and implement treatment plans and functional goals.
• Document treatments and progress notes in patient charts and care plans.
• Coordinate with Interdisciplinary Team to ensure comprehensive care.
• Provide training to family members and staff as part of the care plan.
• Lead program staff and contribute to team meetings and care planning.
