Job summary

Kintegra Health in Charlotte, NC is seeking a Physical Therapist to provide services to Senior TLC participants in a home and community-based model of care. The PT evaluates mobility and functional abilities, prescribes therapeutic exercise and gait training, and develops maintenance programs.

You will report to the Center Manager and collaborate with an Interdisciplinary Team to support quality of life for seniors wishing to live at home.

Qualifications

• Licensed Physical Therapist in NC.
• Valid NC driver's license and vehicle.
• Experience with geriatric population preferred.

Responsibilities

• Conducts initial assessments and periodic reassessments of participants' physical condition.
• Develops treatment programs, sets goals, and provides therapies.
• Records and signs treatments in participants' charts and progress notes.
• Reviews assessments and transfer summaries; plans maintenance and therapy programs.
• Provides training to family members and staff for parts of the plan.
• Leads program aides and participates in Interdisciplinary Team activities.
• Attends staff meetings and updates care plans as needed.
