Job summary

Kintegra Health is seeking a licensed Physical Therapist to provide PT services to Senior TLC participants in a home and community-based care model. The PT will assess mobility and functional status, prescribe therapeutic exercises and gait training, and develop group exercise programs for maintenance therapy.

Reports to the Center Manager and aligns with Senior TLC values of respect, integrity, accountability, and compassionate care.

Qualifications

• Licensed Physical Therapist in NC.
• Valid NC driver's license and vehicle.
• Experience with frail elderly population preferred.

Responsibilities

• Conducts initial and periodic assessments (min 6 months) of participants' physical condition and functional status.
• Develops and implements treatment programs with goals and ongoing adjustments.
• Records treatments in participant charts and progress notes; completes transfer summaries as needed.
• Plans maintenance, exercise, and therapy programs; collaborates with Interdisciplinary Team.
