Job summary

Miami Jewish Health seeks a dedicated professional to develop and implement homecare services for Florida PACE Center participants, coordinating nursing, therapy services and durable medical equipment to ensure high-quality, client-centered care. Responsibilities include assessing needs, coordinating services across disciplines, supervising contracted care, and maintaining compliant documentation.

Qualifications

• Graduate of an accredited school of practical nursing preferred or higher level of education.
• One to three years of experience working with frail elderly population and home care.
• Current LPN license by the Florida State Board of Nursing, preferred
• Current BLS Certification
• Valid Florida Driver’s License and Proof of Insurance

Responsibilities

• Assess homecare needs and develop plans of care.
• Assist in safety evaluations for prospects prior to enrollment.
• Coordinate nursing, physical therapy, occupational therapy, speech therapy, and personal care services.
• Coordinate and authorize Durable Medical Equipment, consumables, and frozen meals.
• Supervise contracted nursing care for participants in facilities.
• Assess and improve homecare services systems for participant needs.
• Participate in Multidisciplinary Team Meetings.
• Monitor compliance of home care agencies and ALF audits.
• Participate in department meetings and in-services.
• Prepare Health Assessment Form 1823 for ALF contracts.
