Job summary

BoldAge PACE is seeking a Care Team Manager to lead the PACE Interdisciplinary Team in East Brunswick, NJ, overseeing care plans and service delivery for participants in a home-based, all-inclusive care program.

The role emphasizes management, coordination, supervision of IDT staff, and development of participant care plans, with a RN preference and a focus on improving quality of life for seniors.

Qualifications

• Minimum of 5 years working in a health care setting.
• 3 years in healthcare-related management position (preferably in a geriatric setting).
• RN license preferred
• 1 year working with the elderly, training on working with a frail or elderly population will be provided upon hire.

Responsibilities

• Manage the PACE Interdisciplinary Team (IDT) and coordinate care plans.
• Facilitate the interdisciplinary team meetings; communicate participant changes and coordinate 24-hour care delivery.
• Ensure IDT interventions are implemented, documented and aligned with regulatory standards.

Education

RN license preferred
