Job summary

Qualifications

• MD or DO with board certification in Internal Medicine or Family Medicine.
• Fellowship training in Geriatric Medicine.
• Current, unrestricted Massachusetts medical license.
• Experience with e-consults or remote specialty support preferred.
• Excellent written and verbal communication; ability to teach clinicians.

Responsibilities

• Provide timely, asynchronous e-consult guidance to primary care clinicians on management of complex older adults.
• Deliver concise, evidence-informed recommendations aligned with ambulatory practice and community resources.
• Apply the 5Ms framework to guide age-friendly care planning in e-consults.
• Arrange one-on-one consultations (virtual or hybrid) when case complexity warrants.
• Support interdisciplinary collaboration and document input in the EHR or e-consult platform.
• Design and lead the Geriatric Education Series to reinforce e-consult learning.
