Qualifications

• Proficiency with common EHRs and telehealth platforms.

Responsibilities

• Provide timely asynchronous e-consult guidance to primary care clinicians on complex older adults.
• Curate and deliver a focused five-part Geriatric Education Series based on Age-Friendly Health Systems 5Ms.
• Arrange one-on-one virtual or hybrid consultations for particularly complex cases.
• Promote interdisciplinary collaboration and clear e-consult communication.
• Document e-consult input and notes within the EHR and assist care transitions.
