Qualifications

• MD or DO with board certification in Internal Medicine or Family Medicine and geriatric fellowship.
• Current Massachusetts medical license.
• Experience with electronic/asynchronous e-consults preferred; clear written guidance essential.
• Strong teaching and communication skills with clinicians.
• Commitment to health equity and serving diverse populations.

Responsibilities

• Provide timely asynchronous e-consult guidance to primary care clinicians on management questions involving complex older adults.
• Deliver concise, evidence-informed recommendations for geriatric care in ambulatory settings.
• Apply the 5Ms Framework to guide age-friendly care planning.
• Arrange one-on-one consultations (virtual or hybrid) for complex cases.
• Lead a focused five-part Geriatric Education Series.
• Promote equity-centered, culturally responsive care.
• Collaborate with care management, behavioral health, and pharmacy as needed.
