A Day in the Life of this Role:

Reporting to the Senior Medical Director for Clinical Services, the Geriatric Physician role centers on primary electronic consultation (e-consult) support to Outer Cape Health Services primary care physicians and advanced practice providers managing complex older adults (age 65+). The Geriatric Consultant delivers timely, high-value, asynchronous e-consult guidance on clinical management questions, enabling primary care teams to address geriatric complexity efficiently within their existing workflows.

When a case is particularly complex and would benefit from direct discussion, a one-on-one consultation with the geriatrician may be arranged, with additional time allocated as needed.

In addition, the Consultant will curate and deliver a focused five-part Geriatric Education Series grounded in the Age-Friendly Health Systems 5Ms Framework (Mind/Mentation, Mobility, Medications, Multi-complexity, and What Matters Most). The series is designed to complement the e-consult service by building primary care providers’ knowledge, confidence, and practical skills in delivering high-quality, age-friendly ambulatory care.

This flexible role is ideal for an experienced geriatrician who values efficient, high-impact e-consult support, selective real-time consultation for complex cases, clinician education, and strengthening primary care capacity in a community health center setting.

Summary of Duties:
Primary Focus:
  • Provide timely, asynchronous electronic consultation (e-consult) to primary care clinicians on management questions involving complex older adults (e.g., multimorbidity, polypharmacy, cognitive impairment/delirium risk, falls and frailty, functional decline, goals of care, and care transitions).
  • Deliver practical, evidence-informed recommendations that are concise, actionable, and aligned with busy ambulatory practice and available Outer Cape community resources.
  • Apply and model the 5Ms Framework in e-consult responses to guide individualized, age-friendly care planning.
  • When a case is complex and would benefit from direct discussion, arrange a one-on-one consultation (virtual or hybrid) with the geriatrician; additional time may be added as needed to support thorough review and collaborative decision-making.
  • Support interdisciplinary collaboration as needed (behavioral health, care management, pharmacy, Councils on Aging) through clear e-consult communication.
  • Document e-consult input (and any one-on-one consultation notes) in a structured, usable format within the electronic health record or designated e-consult platform.
Complementary: Geriatric Education Series
  • Design and lead a focused five-part educational series specifically developed to reinforce and extend the e-consult support provided in this role.
  • Structure content around the Age-Friendly Health Systems 5Ms Framework, emphasizing practical, immediately applicable strategies for primary care.
  • Deliver sessions in flexible formats (hybrid, brief didactic + case discussion, or recorded asynchronous options) that accommodate clinical schedules.
  • Incorporate real-world e-consult themes and cases to strengthen the connection between consultation support and education.
  • Gather feedback and refine the series to maximize impact on provider confidence and clinical practice.
Additional Contributions
  • Promote an equity-centered, culturally responsive approach to the care of older adults.
  • Contribute insights from e-consult patterns to continuous improvement of age-friendly processes within OCHS primary care.
  • Other related duties and projects as assigned.
Qualifications
  • MD or DO with board certification in Internal Medicine or Family Medicine and formal fellowship training in Geriatric Medicine.
  • Current, unrestricted Massachusetts medical license.
  • Demonstrated expertise in the clinical care of complex older adults and application of the Age-Friendly Health Systems 5Ms Framework.
  • Experience providing high-quality electronic or asynchronous consultation (e-consult) preferred; strong ability to deliver clear, concise written clinical guidance essential.
  • Strong teaching and communication skills with experience educating primary care clinicians.
  • Commitment to health equity and serving diverse, underserved, and rural populations.
  • Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
  • Excellent written and verbal communication skills – bi-lingual helpful.
  • Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive, etc.
  • Excellent judgment and ability to solve problems in a timely manner.
  • Ability to adhere to strict confidentiality standards.
  • Demonstrated ability to work effectively both as a team member and independently in a culturally diverse and inclusive environment.
Preferred
  • Prior experience delivering geriatric e-consults or similar remote specialty support to primary care teams.
  • Experience developing and delivering structured educational curricula for clinicians.
  • Familiarity with community health center/FQHC operations, team-based care, and rural healthcare delivery.
  • Comfort with telehealth and electronic consultation platforms.
