Job summary

Hospice of The Bluegrass, Inc. is seeking a Registered Dietitian for the Care Guide Partners PACE program in Elizabethtown, KY. The role focuses on nutrition assessment and education for seniors who require nursing-level care at home.

You will screen nutritional status, plan interventions, and collaborate with the Interdisciplinary Team to ensure meals meet participant needs. Travel within the service area and CPR certification are required.

Qualifications

Licensed Registered Dietitian in Kentucky.
1 year experience with frail/elderly population or training on hire.
CPR certification required.
Willingness to travel within service area.
Proficient with technology-based tools including EMR.
Independent and team player with good communication skills.
Strong organizational skills and ability to multitask.
Knowledge of healthcare, managed care, Medicare/Medicaid regulations.

Responsibilities

Screen and assess participant nutritional status.
Plan and implement nutrition interventions; communicate with Interdisciplinary Team.
Conduct nutritional assessments and provide education.
Coordinate meal services and monitor kitchen facilities.
Coordinate with contracted meal services to meet participants’ needs.
Educate other health care professionals as appropriate.
