Senior Community Care of Kentucky PACE in Florence, KY is seeking a Registered Dietitian for a full-time role. The position runs Monday–Friday, 8:00 a.m.–5:00 p.m., with a $5,000 sign-on bonus and a salary range of $67,400–$74,800 based on experience.

You will assess participants' nutritional needs, develop individualized care plans, provide nutrition counseling, and collaborate with an interdisciplinary care team to support older adults in their communities through the PACE program.

Qualifications

• Current RD/RDN credential.
• Eligibility to practice in Kentucky.
• At least 1 year of clinical nutrition experience with older adults.
• Valid driver''s license if duties require travel.

Responsabilités

• Complete nutrition assessments and develop individualized nutrition care plans.
• Provide nutrition counseling and education to participants and their families.
• Monitor nutritional status, dietary intake, and progress toward care goals.
• Collaborate with the interdisciplinary team to coordinate participant care.
• Support therapeutic diets, menu planning, and dining services as appropriate.
• Maintain accurate clinical documentation and communicate findings to the care team.
