Job summary

Qualifications
- Bachelor's Degree in Nutrition from an ADA-accredited program.
- ADA Registered renewal or current status required.
- One year of experience with frail/elderly population is preferred, but extensive training provided.
Responsibilities
- Perform nutritional assessments and develop individualized care plans.
- Provide nutrition education to participants, caregivers, and staff.
- Collaborate with the interdisciplinary team to coordinate participant care.
- Document care, plans, and observations in the EMR system.
- Monitor progress and adjust nutritional care as needed.
- Ensure compliance with ADA standards and state/federal regulations.
- Participate in team meetings and quality improvement initiatives.
- Assist in planning and evaluating meal services.
