Job summary

PACE Southeast Michigan is seeking a Day Health Center Team Leader/Nursing Assistant to provide supportive services to participants in the day health center, home, and community settings under the supervision of the Day Health Center Manager.

Responsibilities include assisting with personal care, meals, transportation, and activities; coordinating with the RNCM and IDT; maintaining accurate documentation; and supervising center aides while ensuring safety and quality of care for frail elders.

Qualifications

• Must be legally authorized to practice in Michigan.
• Certification required.
• One year nursing assistant experience required.
• Experience with a frail or elderly population.
• Ability to organize and maintain flexible scheduling.

Responsibilities

• Provide personal care and assist with activities.
• Escort to medical appointments and assist as needed.
• Assist with meals, toileting, and incontinence management.
• Medication reminders and supervision under the DHC manager.
• Maintain documentation in the PACE binder and progress notes.
• Support daily operations and coverage for daily appointments.
