Qualifications

• Active, unrestricted licensure in the practicing state.
• Current, active DEA licensure or ability to obtain.
• 1+ years of clinical experience as an APC preferred.

Responsibilities

• Conduct comprehensive assessments for members.
• Support members during rehabilitation episodes.
• Ensure accurate documentation including ICD 10.

Skills

Active ANCC or AANP certification

Current NCCPA certification

Geriatric medicine knowledge

Proficiency with electronic medical records
