Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician to join the Senior Community Care (SCC) team, coordinating care for high-risk health plan members in nursing homes and senior living settings. The role emphasizes comprehensive assessments, documentation, and care planning with an interdisciplinary team.

You will support short-term rehabilitation episodes, communicate with care teams, and educate patients and caregivers while navigating varied environments.

Qualifications

• Active, unrestricted driver's license
• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current NCCPA certification and state licensure
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire (Not needed for Patient Connect)
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments and to position oneself to perform job duties

Responsibilities

• Conduct comprehensive assessments including medical history review, vitals, and physical exam
• Support members during a short term sub-acute rehabilitation episode
• Ensure accurate and complete documentation including ICD-10
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education
