Job summary

UnitedHealth Group's Optum Home & Community Care is hiring Advanced Practice Clinicians to join the Senior Community Care program in Washington. You will assess, treat, and coordinate care for high-risk members in nursing homes, senior housing, and home settings.

No on-call, no weekends, and no holidays required. Compensation ranges from $109,500 to $164,000 annually, plus sign-on, loan repayment, and relocation bonuses, with a comprehensive benefits package.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: NCCPA certification and state licensure
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without disciplinary actions
• Active and unrestricted driver's license with reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain per state regulations
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to navigate varied environments and to position oneself to perform job duties

Responsibilities

• Conduct comprehensive assessments including history review, reconciliation, vitals and exams
• Support members during short-term sub-acute rehab episodes
• Ensure accurate documentation including ICD-10 codes
• Communicate and collaborate with the interdisciplinary care team
• Lead advanced illness and end-of-life discussions
• Provide counseling and education to patients and caregivers
