Job summary

Your Health Organization in the Gainesville area seeks a Cognitive Behavioral Specialist to support patients with anxiety, depression, and cognitive decline, including dementia. The CBS works within an interdisciplinary care team, providing structured behavioral interventions and promoting caregiver support to align with medical treatment for a patient-centered approach.

The role includes visits in facilities and patients’ homes in the designated service area, with daily travel and a full-time,

Qualifications

• Must have minimum five (5) years of experience with dementia or cognitive impairments.
• Experience in geriatrics or memory care settings; if licensed, license must be in good standing.
• Willingness to enroll in a Dementia certification course.
• Familiarity with behavioral health medications and interventions for this population.
• Strong reading, writing, and communication skills.
• Ability to read and communicate effectively; basic computer knowledge.

Responsibilities

• Manage a caseload of patients diagnosed with cognitive behaviors.
• Conduct patient visits on a cadence tailored to patient needs (weekly/biweekly/monthly).
• Observe behavioral patterns across times of day; maintain detailed documentation.
• Implement structured behavioral protocols to assess and address behavior.
• Monitor changes in behavior to adjust care plans.
• Collaborate with primary and specialty providers for aligned plans.
• Participate in interdisciplinary case reviews and telehealth.
• Educate and support caregivers in identifying and managing triggers.
• Lead caregiver coaching sessions on behavioral modification and communication.
• Lead family and community education initiatives on dementia and aging.
