Your Health in Brunswick, GA is seeking a Cognitive Behavioral Specialist to support patients with anxiety, depression, and dementia within facilities and home settings. You will provide structured behavioral interventions and collaborate with an interdisciplinary care team to ensure comprehensive, patient-centered care.

This full-time role requires reliable transportation for daily travel between offices, facilities, and patient homes.

Qualifications

• Minimum of five years of experience with dementia or cognitive impairments.
• Experience in geriatrics or memory care settings (Activities Director, CNA, MA, RN/LPN). If licensed, license in good standing.
• Actively enrolled in or willing to enroll in dementia certification.
• Dementia care certification preferred or willingness to pursue certification.
• Familiarity with behavioral health medications and interventions for this population.
• Strong written and verbal communication.
• Basic computer knowledge.
• Ability to lead and collaborate in a team environment.
• Must travel between offices and facilities; valid driver's license required.

Responsibilities

• Manage a caseload of patients diagnosed with cognitive behaviors.
• Conduct visits at a cadence tailored to patient needs (weekly, then biweekly, then monthly).
• Observe behavioral patterns and maintain detailed documentation.
• Implement a structured behavioral protocol and address triggers.
• Monitor changes to adjust care plans.
• Collaborate with primary and specialty providers for aligned plans.
• Participate in interdisciplinary case reviews and telehealth consults.
• Educate caregivers in managing triggers and strategies.
• Lead education initiatives for families and communities on dementia and aging.
