Your Health seeks a Cognitive Behavioral Specialist to support patients in the Gainesville area. The CBS will work within an interdisciplinary team, addressing anxiety, depression, and cognitive decline across facilities and homes, with structured interventions and caregiver support.

Responsibilities include visiting patients on a set cadence, documenting behaviors, and coordinating with providers to optimize care. Travel is required daily, and the role is full-time with 8-hour shifts.

Qualifications

• Minimum of five years of dementia or cognitive impairment care experience.
• License in good standing if applicable.
• Willingness to enroll in a dementia certification course.

Responsibilities

• Manage a caseload of patients with cognitive behaviors.
• Conduct patient visits on a scheduled cadence, including weekly/biweekly follow-ups.
• Observe behaviors and maintain detailed documentation.
• Implement structured behavioral protocol and address triggers.
• Monitor changes to adjust care plans.
• Collaborate with primary and specialty providers for aligned plans.
• Educate caregivers and provide coaching on behavioral strategies.
• Lead education initiatives for families and the community.
