Your Health in Marietta, GA is seeking a Cognitive Behavioral Specialist to support patients with anxiety, depression, and cognitive decline in facilities and homes. You will travel daily within the service area, perform structured behavioral interventions, and collaborate with the care team to align medical and behavioral plans.

The role requires at least five years of dementia experience, potential certification, and strong communication and leadership skills.

Competenze

• Minimum of five (5) years of experience with dementia or cognitive impairments.
• Experience in geriatrics/memory care settings.
• Active enrollment in or willingness to enroll in a dementia certification course.
• Familiarity with behavioral health medications and interventions.
• Ability to read and communicate effectively; strong written and verbal skills.

Mansioni

• Manage a caseload of patients with cognitive behaviors.
• Conduct patient visits on a schedule tailored to patient needs (weekly, biweekly, monthly).
• Observe behavioral patterns and maintain detailed documentation.
• Implement structured behavioral protocols and address triggers.
• Monitor changes to adjust care plans as needed.
• Collaborate with primary care and specialists to align plans.
• Participate in interdisciplinary reviews and telehealth consults.
• Educate caregivers and provide coaching on behavioral strategies.
• Lead or participate in education initiatives for families and community.
