Your Health, a physician-led care group serving GA and SC, seeks a Cognitive Behavioral Specialist to support patients with anxiety, depression, and cognitive decline in facilities and homes. The CBS will work with an interdisciplinary team, promote caregiver support, and align behavioral management with medical treatment.

Qualifications

• Minimum of five (5) years of experience working with individuals with dementia or cognitive impairments.
• Experience in geriatrics or memory care settings; license in good standing if licensed.
• Enrolled in or willing to enroll in a Dementia certification course.

Responsabilités

• Manage a caseload of patients with cognitive behaviors.
• Conduct visits weekly, biweekly, then monthly as appropriate.
• Observe behavior patterns and document details diligently.
• Apply a structured behavioral protocol and identify triggers.
• Coordinate with primary and specialty care for aligned plans.
• Participate in interdisciplinary reviews and telehealth consults.
• Educate caregivers and support prevention of triggers.
• Lead caregiver coaching on behavioral modification.
• Lead education for families on dementia and aging.
