Job summary

Your Health Organization is seeking a dedicated Cognitive Behavioral Specialist to support patients with anxiety, depression, and cognitive decline in the Gainesville area. The CBS will work within an interdisciplinary team to deliver structured behavioral interventions and caregiver support, aligning strategies with medical treatment for comprehensive, patient-centered care.

Qualifications

• Minimum of five (5) years of experience working with individuals with dementia or cognitive impairments.
• Active driving license with reliable transportation is required.
• Willingness to enroll in a Dementia certification course if not already certified.
• Familiarity with behavioral health medications for this population.
• Strong written and verbal communication skills and ability to lead.

Responsibilities

• Manage a caseload of patients diagnosed with cognitive behaviors.
• Conduct patient visits in facilities or homes in the designated service area.
• Document behavioral observations and maintain detailed records.
• Implement structured behavioral protocols and caregiver coaching sessions.
• Collaborate with primary and specialty providers for integrated care plans.
• Lead education initiatives for families and caregivers on dementia and aging.
• Travel between offices, facilities, and patient homes daily.
