Job summary

Your Health Organization in the Brunswick area seeks a Cognitive Behavioral Specialist to support patients with anxiety, depression, and cognitive decline. You will work within an interdisciplinary care team delivering structured behavioral interventions, caregiver support, and alignment with medical treatment.

Travel daily to facilities and patients' homes as needed; valid transportation required. This 8-hour shift, Monday–Friday, 8:00 AM–5:00 PM, offers competitive pay and benefits including

Qualifications

• Minimum of five (5) years of experience with dementia or cognitive impairments.
• Experience as Activities Director, CNA, MA, RN/LPN or similar in geriatrics/memory care.
• Enrolled in or willing to enroll in a dementia certification course.
• Dementia care certification preferred or willingness to pursue certification.
• Familiarity with behavioral health medications and interventions for this population.
• Able to read and communicate effectively; strong written and verbal skills.
• Basic computer knowledge; able to use standard software.
• Leadership skills with ability to foster a cooperative team environment.

Responsibilities

• Manage a caseload of patients diagnosed with cognitive behaviors.
• Conduct patient visits at a cadence tailored to patient needs (weekly/biweekly/monthly).
• Observe behavioral patterns and maintain detailed documentation.
• Implement structured behavioral protocols and address triggers.
• Monitor changes to adjust care plans; coordinate with PCP/specialists.
• Participate in interdisciplinary case reviews and telehealth consults.
• Educate and support caregivers on triggers and strategies.
• Lead caregiver coaching sessions on modification and communication.
• Participate in family/community education on dementia and aging.
