Job summary

AltaMed is seeking a Licensed Clinical Social Worker in Pico Rivera to provide psychosocial assessments and psychological interventions to PACE participants. The role focuses on collaborating with care teams to address behavioral health issues and ensure participant care as outlined on the care plan.

Ideal candidates will possess a Master's in social work and have at least 2 years of experience with elderly or disabled adults.

Qualifications

• Minimum of 2 years of experience with elderly or disabled adults and/or mental health/substance abuse populations.
• A minimum requirement of a valid BLS certification or higher.
• Experience providing individual psychotherapy and/or leading group therapy sessions.

Responsibilities

• Provide psychosocial assessments and psychological interventions to PACE participants.
• Conduct consultations with care teams about behavioral health issues.
• Coordinate service delivery and assessment/treatment recommendations.
