Job summary

Partners in Care Foundation is seeking an experienced social worker to assess and plan care for elderly clients in the home setting about independence and wellbeing. The role requires collaborating with a multidisciplinary team and leveraging community resources to provide comprehensive support.

You will conduct psychosocial assessments, develop tailored care plans, and ensure ongoing monitoring while documenting all processes per policy.

Qualifications

• Bachelor’s degree in a related field is required.
• Minimum two years of experience working with the elderly.
• Ability to perform clinical assessments and engage with a multidisciplinary team.
• Knowledge of aging and programs for seniors.

Responsibilities

• Evaluate the client as a whole to identify functional limitations affecting independence.
• Perform in-depth psychosocial, rehabilitation, and environmental assessments.
• Refer clients to specialists for detailed evaluations and recommendations.
• Develop and implement the care plan with follow-up and monitoring.
• Identify and develop support systems for clients.
• Consult with Health Practitioners as needed.
• Document case records and reporting.
• Provide case management per departmental policy and client needs.
• Complete assessments of health and psychosocial status per policy.
• Participate in care planning to tailor plans to client needs.
• Utilize community resources to implement interventions promptly.
• Maintain regular contact with clients as required.
