Job summary

TheKey is recruiting a Geriatric Care Manager to provide holistic, client-centered care for aging clients at home. You will assess needs, develop comprehensive care plans, and coordinate services to preserve dignity and independence.

Responsibilities include managing medical and cognitive challenges, collaborating with families and professionals, and maintaining electronic health records and billing. Strong communication and organizational skills are essential.

Qualifications

• 5+ years’ experience in community, hospital, hospice or home health case management.
• Bachelor’s or master’s degree in healthcare related field preferred.
• Certified Care Manager (CMC) or Certified Case Manager (CCM).
• Demonstrated knowledge of aging, dementia, and/or disabilities.
• Experience working with complicated family systems and dynamics.
• Computer skills: MS Office, EHRs and Internet skills.
• Excellent customer service and problem-solving skills.
• Excellent verbal and writing skills.
• Detail-oriented with the ability to keep up with fast-paced processes.

Responsibilities

• Develop comprehensive care plans and implement them.
• Track, assess and document client status, actions and outcomes.
• Monitor and report on client status and interventions.
• Coordinate resources for client care with families and professionals.
• Interact with physicians, attorneys, fiduciaries, hospice and community staff.
