Job summary

Care Connection for Aging Services is seeking a Care Manager to deliver individualized case management for older adults across the service area. You will assess needs, advocate, screen for public benefits, and connect clients with community resources to promote independence and quality of life.

The role requires extensive computer use, strong data entry skills, and regular client interactions in office and home environments. Travel is required; SHIP/SMP/AIRS certifications are preferred.

Qualifications

• Bachelor's degree or equivalent combinations of education and relevant work experience may be considered.
• Strong verbal and written communication skills with diverse audiences.
• Proficiency in computer applications and data entry; familiarity with data and case management systems.
• Must have reliable transportation, valid driver's license, and proof of automobile insurance.
• 2 years' experience in related field preferred.

Responsibilities

• Assess client needs and develop individualized care plans.
• Provide advocacy and support to older adults and families.
• Screen clients for eligibility for public benefits and assist with applications.
• Maintain accurate records across multiple data and case management systems.
• Connect clients with community resources and services.
• Assist with programs that promote independence, health, and quality of life.
• Engage with clients in office settings and diverse home environments.
• Maintain professional certifications (SHIP, SMP, AIRS).
• Manage multiple priorities in a fast-paced environment.
• Travel moderately within and outside the service area as needed.
